INSTRUCTIONS
The below application form you
see is nothing more
than a photograph
in the format of a .jpg
All you need to do is to "right click" anywhere on the
form and save it to your computer. Once you have
done this all you need to do then is to print it out
with your printer. Then, simply fill it out and mail
it in to the Association.
There is no membership fee however donations
are always appreciated.

To bring up the order form, simply
click on the
PDF button below.
This will open up the order form.
Once
you have done that you should see a small
icon
in the upper left hand corner that looks like a
little
printer. You can click on that to print out your
order
form.
You
can also click on the word "File" in the very upper
left corner of your screen. This should give you a drop
down navigation bar ..... scroll down to print and click it.
